Our dentists at Smiles Forever want you to receive the treatment and services that you need and desire without having to stress about finances or payment for treatments. We understand dental costs can be overwhelming, but we are here to help you with it!
We are in-network providers with Delta Dental Premier, Blue Cross Blue Shield. For your convenience, we also offer complimentary insurance billing and claim submission.
For your out of pocket expenses or Copays, our office accepts the following:
Cash, Credit cards ( MasterCard, Visa, American Express), Flexible Spending Account cards, Health Savings Account cards.
*Please note- We do not accept personal checks.
We also provide interest-free affordable monthly payment plans through Care-Credit*. So don’t hesitate to ask us, if you need more information.

In Office Dental Care Plan:
It is our goal to provide you treatment which helps to improve or maintain your dental health and well being. Therefore, aside from accepting insurances and offering third party financing, we are also providing our patients with different types of In-House Yearly Membership Plans.

Plan 1 = $500 ( For Adult Patients)
This Plan includes following:
- Dental Cleanings 2 times in your plan year (Regularly $290)
- Dental Exams with Dentists 2 times in your plan year (Regularly $156)
- Necessary radiographs updated in your plan year (Regularly $182)
- Fluoride varnish 2 times in your plan year (Regularly $126)
- Free One Emergency Visit Exam and Necessary Radiograph (Regularly $132)
- Discounted FMX series (Once/5 years) for a fee of 135$ (Regularly $180)
- 10% discount on all treatment services
Plan 2 = $750 (For Adult Patients)
This Plan includes following:
- Dental Cleanings 2 times in your plan year (Regularly $290)
- Periodontal Cleanings 2 times in your plan year (Regularly $384)
- Dental Exams with Dentists 2 times in your plan year (Regularly $156)
- Necessary radiographs updated in your plan year (Regularly $182)
- Fluoride varnish 2 times in your plan year (Regularly $126)
- Free One Emergency Visit Exam and Necessary Radiograph (Regularly $132)
- Discounted FMX series (Once/5 years) for a fee of $135 (Regularly $180)
- 10% discount on all services
Plan 3 = $350 (For our Child Patients)
- Dental Cleanings 2 times in your plan year (Regularly $210)
- Dental Exams with Dentists 2 times in your plan year (Regularly $156)
- Necessary radiographs updated in your plan year (Regularly $142)
- Fluoride varnish 2 times in your plan year (Regularly $126)
- Free One Emergency Visit Exam and Necessary Radiograph (Regularly $132)
- 10% discount on all treatment services
Limitations and Exclusions
- Membership fee to be paid in full on the day of enrollment.
- Membership Valid for only 1 year from Day of Plan Purchase. Unused benefits may not be rolled over to another membership year.
- NO REFUNDS on membership fees.
- Membership/Unused benefits are Non-transferable to other family members.
- We require 48 Hours Notice for Cancellations or Rescheduling of appointments due to any reason.
- No shows/ less then 48 hours notice will result in VOID of your annual membership prevention plan.
- Fees for single appointment treatment due in full on date of service.
- Minimum 50% Fees due on the first appointment for treatment needing multiple appointments.
